Cyber addiction and ADHD as a global new syn-

drome in adolescents

Chair: O. Bilke-Hentsch, Switzerland

Description

Eighty to ninety percent of all children and adolescents

enjoy the use of modern electronic interactive media and

the web 2.0 as well as social networks to enrich their

lives, although parents and teachers sometimes have

got different ideas of meaningful activity. On the other

end of the spectrum pathological Internet and media

use (“PIU”) seems to emerge as an important public

mental health problem in children and adolescents of

risc populations e.g. with ADHD, learning disorders and

social phobia.

After 20 years of clinical and basic research on the

side-effects of inappropriate use the problem has been

partly considered in DSM5 as a disease entity (Internet

gaming disorder) since 2013. Although the concept is

still discussed controversly by addiction therapists, there

exist clinical and neurobiological findings, that PIU and

its severe addiction form resembles classical adolescent

addiction patterns such as gambling, marihuana or

alcohol dependency.

There seems to be a small but severely ill subgroup of

multiple media users with comorbidity and multiple risk

factors that develop pervasive social isolation, increase of

dosage, craving and other features of addiction.

This is important in particular for ADHD patients, because

their core symptoms are excessively rewarded by several

Internet and media applications.

In this seminar recent diagnostic and therapeutic

concepts will be discussed in order to encourage ADHD

therapists to recognize PIU as a major problem in an

increasing subgroup of ADHD patients.

Using the clinical diagnosis group PIU, in the Swiss

Kanton Zuerich a comorbidity-oriented out- and in-

patient approach has been established on a hospital-

based level. Psychoeducation, neuropsychological

diagnosis, counselling of parents and teachers and

treating severe individual cases in a three-step approach

(SOMOSA-Medialab) are some parts of the programme.

Diagnostic process and treatment planning for

children

Chairs: M. Stein, USA, I. Manor, Israel

Description

We will review the diagnostic process using several

case example of simple, subtle, and complex ADHD,

and provide a model for providing feedback and linking

diagnostic findings to treatment. Strategies for identifying

symptoms, comorbidities, impairments, comorbidities,

and strengths (SIMMS) will be reviewed as well as

diagnostic dilemmas and controversies, such as limitations

of behavior rating scales, use of laboratory measures, and

the role of psychological/neuropsychological tests.

ADHD treatment in pregnancy and child outcomes

Chair: D. Silva, Australia

Description

ADHD is being more recognised and treated across the

globe with an understanding that ADHD symptoms do

not just disappear in adolescents but may need regular

treatment into adulthood. There are more females

of childbearing age required to make a decision on

either discontinuing, reducing the dose or continuing

medication to treat their ADHD. The advice of their

doctors and specialists is important and needs to be

delivered with an understanding of all the evidence to

date. Unfortunately, this evidence is limited. The U.S.

Food and Drug Administration has classified ADHD

medications as being "pregnancy category C", implying

that there is insufficient information to confirm either

harm or lack of harm. Mouse studies have shown

the teratogenicity of MPH but not in small human

studies. However, there is evidence of some increase in

miscarriage rates and lower birth weight.

Currently, there are no adequate guidelines for the

treatment of ADHD across pregnancy and in the

postpartum period. This education session will provide

an understanding of the current evidence, suggested

patient management and future studies.
